Application form and annexes

1. What is the Legal entity sheet, the Financial identification form? How it can be in Ukrainian, does Ukraine have some equivalent of these documents?

   Legal entity sheet and the Financial Identification form are the supporting documents which should be submitted together with the Application Form and its annexes as defined in the point 2.2.1 of the Guidelines for the applicants “Application Form, its annexes and supporting documents”.

   These documents where elaborated by the European Commission intended to help you in communicating to the Contracting Authority. 

   Legal entity sheet has to be filled in according to your legal statute providing the Contracting authority with the following information: type of the company, name, official address, contacts, place and date of the registration.

   Financial identification form helps the Contacting authority to communicate your banking co-ordinates in particular your bank account you will use for the project’s needs. As regards bank accounts it is very important to make sure that the title and the address of the account that you will mention on the Form are exactly in conformity with what is recorded at your bank.

   The Legal entity sheet and the Financial Identification form must be signed by the authorized representative of Applicant’s organization and stamped.
